Zara Anne Radcliffe, 30, pleaded guilty to the murder of John Rees, 88, before the Crown Court of Merthyr in South Wales.
Mr. Rees was stabbed to death in a cooperative shop on May 5 in a series of attacks in the town of Pen Y Graig, Rhondda. Radcliffe also admitted to the assassination attempts of Andrew Price, 58, Gaynor Saurin and Lisa Way, 53, all of whom were injured in the incident.
She appeared in a video link from Rampton High Security Hospital in Nottinghamshire, where she is currently being held.
Prosecutor Michael Jones QC told the court that the pleas were acceptable to the Crown as Radcliffe was suffering from schizophrenia at the time of the incident.
Radcliffe will be sentenced Wednesday.
An earlier investigation revealed that Mr. Rees, who lived with his wife Eunice, 87, in the nearby village of Trealaw, died at the scene of severe blunt force trauma to the face, including multiple facial fractures.
Radcliffe attacked him in the store while his wife waited outside in their car.
She suffers from dementia and keeps asking, “Where is John?”
In a statement after his death, his family said: “John was the definition of a good man who was extremely respected and loved in the community.
“He was proud of his family, proud to be Welsh and dedicated to All Saints Church.
“We will all miss him very much.”
Flowers were left at All Saints Church in nearby Trealaw, where John was principal and his wife Eunice was a member of the ward.

John rang the chapel bell every Thursday when it came to NHS workers.
Nephew Michael Smith said: “My aunt is extremely fragile. Eunice was totally dependent on him.
“Although John was 88 years old, he was in good physical shape and often took him to Pontypridd and they would sit at Prince’s Cafe.
“We think so highly of him. Everyone knows John. If you can draw inspiration from it, you will be fine.
“If you try to think about what I would do in a situation and you do, you are on the right track.”
John’s family paid tribute to him, saying he would “do anything for everyone” and would help the other victims of the knife attack if he died.
Lisa Way, an NHS worker, was also injured in the attack and left a message on the flowers: “Dear John, I’m sorry I can’t save you. I hope you are at peace now. ”
Radcliffe is the mother of a young son and was recently discharged from the mental health department prior to the attacks.
A romantic relationship with a man had ended the day before the murder and Radcliffe had filed a complaint with the police.
